  1. Eligibility: 

This Campaign is open only to those who [meet your age, location and other requirements] 

2. Agreement to Rules: 

By participating, the Contestant (“You”) agree to be fully unconditionally bound by these Rules, and You represent and warrant that You meet the eligibility requirements. In addition, You agree to accept the decisions of Cheshire Dental Centre as final and binding as it relates to the content of this Campaign. 

  1. Campaign Period: 

Entries will be accepted online starting 14th February 2022 on and ending 18th February 2022. All online entries must be received by 18th February 2022. 

  1. How to Enter: 

The entry must fulfil all Campaign requirements, as specified, to be eligible to win a prize. Entries that are incomplete or do not adhere to the rules or specifications may be disqualified at the sole discretion of Cheshire Dental Centre. 

Optional verbiage to include: 

You may submit two entries only. You must provide the information requested. You may not enter more times than indicated by using multiple email addresses, identities, or devices in an attempt to circumvent the rules. If You use fraudulent methods or otherwise attempt to circumvent the rules, your submission may be removed from eligibility at the sole discretion of Cheshire Dental Centre. 

  1. Prizes: 

The Winner of the Campaign (the “Winner”) will receive £500 towards the cost of a Smile Makeover which MUST include treatment for a value of £2500.00. The specifics of the prize shall be solely determined by Cheshire Dental Centre. No cash or other prize substitution. The prize is non-transferable. No substitution of prize or transfer/assignment of prize to others or request for the cash equivalent by Winner is permitted. 

6. Odds: 

The odds of winning depend on the number of eligible entries received. 

7. Winner Selection and Notification: 

Winner will be selected under the supervision of Cheshire Dental Centre]. Winner will be notified by email only within 10 working days following selection of Winner. Cheshire Dental Centre shall have no liability for Winner’s failure to receive notices due to spam, junk e-mail or other security settings or for Winner’s provision of incorrect or otherwise non-functioning contact information. If Winner cannot be contacted, is ineligible, fails to claim the prize within 30 days from the time award notification was sent, or fails to timely return a completed and executed declaration and release as required, the prize may be forfeited and an alternate Winner selected. ANY VIOLATION OF THESE OFFICIAL RULES BY WINNER WILL RESULT IN WINNER’S DISQUALIFICATION AS WINNER OF THE CAMPAIGN, AND ALL PRIVILEGES AS WINNER WILL BE IMMEDIATELY TERMINATED. 

8. Rights Granted by You: 

By entering this content (e.g., photo, video, text, etc.), You understand and agree that Cheshire Dental Centre or anyone acting on behalf of [Cheshire Dental Centre and Cheshire Dental Centre licensees, successors, and assigns, shall have the right, where permitted by law, to print, publish, broadcast, distribute, and use in any media now known or hereafter developed, in perpetuity and throughout the World, without limitation, your entry, name, portrait, picture, voice, likeness, image, statements about the Campaign, and biographical information for news, publicity, information, trade, advertising, public relations, and promotional purposes. without any further compensation, notice, review, or consent. 

9. Terms & Conditions: 

Cheshire Dental Centre reserves the right, in its sole discretion, to cancel, terminate, modify or suspend the Campaign should virus, bug, non-authorized human intervention, fraud, or other cause beyond Cheshire Dental Centre control corrupt or affect the administration, security, fairness, or proper conduct of the Campaign. In such case, Cheshire Dental Centre may select the Winner from all eligible entries received prior to and/or after (if appropriate) the action taken by Cheshire Dental Centre. Cheshire Dental Centre reserves the right, in its sole discretion, to disqualify any individual who tampers or attempts to tamper with the entry process or the operation of the Campaign or website or violates these Terms & Conditions. Cheshire Dental Centre has the right, in its sole discretion, to maintain the integrity of the Campaign, to void votes for any reason, including, but not limited to: multiple entries from the same user from different IP addresses; multiple entries from the same computer in excess of that allowed by Campaign rules; or the use of bots, macros, scripts, or other technical means for entering. Any attempt by an entrant to deliberately damage any website or undermine the legitimate operation of the Campaign may be a violation of criminal and civil laws. 

10. Limitation of Liability:

By entering, You agree to release and hold harmless Cheshire Dental Centre and its subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers, and directors from any liability, illness, injury, death, loss, litigation, claim, or damage that may occur, directly or indirectly, whether caused by negligence or not, from: 

  1. such entrant’s participation in the Campaign and/or his/her acceptance, possession, use, or misuse of any prize or any portion thereof; 
  2. technical failures of any kind, including but not limited to the malfunction of any computer, cable, network, hardware, or software, or other mechanical equipment;
  3. the unavailability or inaccessibility of any transmissions, telephone, or Internet service;
  4. unauthorized human intervention in any part of the entry process or the Promotion; 
  5. electronic or human error in the administration of the Promotion or the processing of entries. 

11. Disputes: 

As a condition of participating in this Campaign, participant agrees that any and all disputes that cannot be resolved between the parties, and causes of action arising out of or connected with this Campaign, shall be resolved individually, without resort to any form of class action, exclusively before a court located in United Kingdom having jurisdiction. Further, in any such dispute, under no circumstances shall participant be permitted to obtain awards for, and hereby waives all rights to, punitive, incidental, or consequential damages, including reasonable attorney’s fees, other than participant’s actual out-of-pocket expenses (i.e. costs associated with entering this Campaign). Participant further waives all rights to have damages multiplied or increased. 

12. Privacy Policy: Information submitted with an entry is subject to the Privacy Policy stated on the Cheshire Dental Centre] website. 

13. Sponsor: The Sponsor of the Campaign is Cheshire Dental Centre.

Treatment plans and Estimates

Once your treatment plan has been agreed with the Dentist, we will provide you with a printed treatment plan that we require you to sign. If you would like a copy of your signed treatment plan then please ask a member of our staff and we will happily provide this for you. If your treatment plan changes due to radiographic or clinical findings, we will inform you and discuss this with you and ask you to sign a revised copy. If there are any points on your treatment plan that you wish to query, please do not hesitate to ask us.

Consent forms

Certain treatments require completion of a written consent form. This is in order to explain the treatment, aftercare, and any risk to you thoroughly before any of these treatments are carried out.

Consent for any treatment can be withdrawn at any point by the patient or dental professional.

Fees

We require pre-payment and deposits to secure the booking of appointments. We do not operate a credit system and we require fees to be settled at the appointment where treatment is provided. Where treatment incurs laboratory fees, the full fee will be required at the impression stage of the appointment.

Cheshire Dental Centre does offer finance agreements on treatments over £300 in association through a third party for patients depending on the clinical case. In cases where treatment is paid for by finance through a finance company, the finance agreement must be accepted and signed prior to the start of that treatment, including waiting for the period that the cooling offer period may apply.

Patients registered to us with Denplan must see their terms and conditions for treatments under contract. 

Cheshire Dental Centre does not accept any payments by cheque.

Payment methods accepted are:

  • Cash
  • Debit/Credit Card excluding American express

Please note that unpaid accounts are routinely referred to a Debt Collection Agency or the Small Claims Court, and we reserve the right to recover all costs incurred in doing so.

We reserve the right to ask for payment in full before beginning any treatment plan.

Late cancellation or missed appointments:

We require a minimum of 48 business hours notice for cancellation of any arranged appointment. When we schedule an appointment for a patient, we are booking the surgery time off for that patient’s treatment. Failure to give the appropriate notice of cancellation will result in a Failed Appointment Fee being charged. This fee is proportionate to the length of the appointment failed and is to cover the cost of the surgery time wasted.

Failed Appointment fees must be settled before any other appointment is offered. 

Cheshire Dental Centre reserves the right to charge the full cost of the treatment in the event of a missed appointment or an appointment cancelled with less than 24 hours’ notice. 

For missed appointments or short notice cancellations, we reserve the right to take a minimum of 50% deposit to book further appointments. 

Please note it is also down to Cheshire Dental Centres discretion if further appointments can be booked for any late cancellations or missed appointments.

Late for appointments:

We understand that some patients travel long distances to get to the clinic, and in some cases being late for appointments can be unavoidable. If you are more than 10 minutes, please be aware that you may be asked to reschedule your appointment.

Personal Details:

It is very important that you give a full medical history and details of any medication you take. Should these change in any way, it is very important for you to tell your Dentist. It is the patient’s responsibility to inform the clinic of any changes in either personal details and/or their medical history.

Guarantee:

At Cheshire Dental Centre we offer a 12-month guarantee for fillings, and crown and bridgework, provided that the following conditions are met:

  1. The patient has fully paid for the treatment and does not owe the practice any money for the treatment received.
  2. The restorations have not been damaged as a result of an accident, trauma, or excessive grinding.
  3. The patient has followed all post treatment maintenance recommendations made by our dentists.
  4. The patient has attended and routine examination every 6 months.

Some treatments may have a guarantee of less than 1 year, and in this case, you will be informed by your Dentist either verbally or in writing, or both.

Use of Images and X-rays:

Cheshire Dental Centre may use images and x-rays of your smile and teeth only, for marketing and educational purposes on the Cheshire Dental Centre website, social media and on promotional and educational literature. Your name will never be published, and your identity will never be disclosed. However, if you DO NOT wish for us to use your images and x-rays in this way, please let us know. We will always ask you to sign a consent form for dental photographs to be used prior to your treatment.

Use of patient contact details:

At Cheshire Dental Centre the health of our patients is our highest priority, and we also like to keep our patients informed of various important changes at the clinic and of our latest special offers. We like to remind our patients of their appointments, when they are due for appointments, and other various important reminders. On this note, you may be periodically contacted by the clinic via phone, text, email or by letter in the post. If you DO NOT wish to be contacted by the clinic by any or all of these means, please let us know.

NHS Patients

Cheshire Dental Centre has a very small NHS contract as we are predominately a Private Dental Practice, therefore we only have limited availability. Unfortunately, many of our patients forget to make regular visits to see their dentist and then, before they realise, it’s been over 18 months. Due to the high number of people wishing to receive NHS dental treatment we have no choice but to remove patients who have not attended for 18 months or more from the NHS list.

Cheshire Dental Centre tries to remind our patients when they are due for their dental examinations via text message, email, or letters. However, we often find that many of our communications get returned as patients do not update their details with us. It is ultimately the patient’s responsibility to keep up to date with their appointments and to attend every six months unless the dentist has stated otherwise.

Complaint’s policy:

At Cheshire Dental Centre we always take complaints about any aspects of our services very seriously, in order to ensure that every patient has only the very best experience at all times.

Complaints can be made in writing by either writing to our Practice Manager or emailing emma.cheshiredentalcentre@gmail.com

Every complaint will receive immediate written/email acknowledgement, and the Practice Manager will strive to resolve the complaint within a quick, reasonable period of time. For our full complaints procedure and policy, please ask a member of our reception team. If a complaint is about any aspect of clinical care or associated charges it will normally be acknowledged by the Practice Manager and then referred to the treating Dentist. Please note that in these cases, Cheshire Dental Centre accepts no liability on behalf of the treating Dentist and acts as a liaison between the patient and the treating Dentist, only.

If you are unsatisfied with our internal handling of your complaint, you are free to forward your complaint to the Dental Complaints Service (www.dentalcomplaints.org.uk). If still unsatisfied, you may wish to forward your complaint to the General Dental Council.

No tolerance/Abuse policy

We operate a zero-tolerance policy to abuse our Dentists and staff, loud/disorderly/drunken behaviour, persistent missing and late cancellation of appointments (after multiple warnings). In these situations, Cheshire Dental Centre reserves the right to refuse treatment and admission.

Promotions

Cheshire Dental Centre occasionally runs special offers and promotions on treatments. These are subject to the availability of appointments and the suitability of the treatment for the patient.

Data Protection Act:

We store all patient personal details on a secure computer system in accordance with the Data Protection Act & GDPR. All clinical notes, digital radiographs, digital photographs etc remain the property of Cheshire Dental Centre. 

We would also remind patients that we are not able to discuss a patient treatment or appointments with a family member or anyone else without written consent from the patient under current GDPR regulations.

Copies of notes, radiographs and photographs can be made available to patients but we would need this in writing beforehand.
